Adult Participant Waiver

Participants age 18 and older can review the waiver below and complete the electronically signed form through PlayMetrics.

UNDER AGE 18

Minor Participant Waiver

A parent or legal guardian must complete the minor participant waiver. Review the PDF or use the button below to sign online through PlayMetrics.

Complete your waiver before your first game. All players must complete the appropriate waiver. If you play on multiple teams or in more than one MSA league, you only need to complete it once for the current MSA league year.
